How to Download Forms

Internet Explorer Users:
To download a form, right-click the format of the file you desire to download. From the menu that appears, select "Save Target As...". You will be given a window to select a location to download the file, such as your Desktop.

Netscape Navigator User:
To download a form, right-click the format of the file you desire to download. From the menu that appears, select "Save Link As". You will be given a window to select a location to download the file, such as your Desktop. Some problems have been experienced in downloading WordPerfect forms with Netscape Navigator (specifically, the file name is modified during the download to include one or more "%" characters, and the downloaded file cannot be opened in WordPerfect); please try Internet Explorer if you experience any problems.

Each of the forms below was tested on A4 paper. If you experience any technical problem with the forms, then please email to the Firm a brief description of the problem and attach the relevant form to your email.

Declaration - Power of Attorney
signed by the inventor is required for every case, but may be filed late with a surcharge.

page is required if there is more than one inventor.

Application Data Sheet (ADS)
You may complete the ADS, or the Firm may complete the ADS based on information that you provide to the Firm.

Revocation of Power of Attorney and Appointment of New Attorney
This form is to transfer to the Firm, an active U.S. patent application which is currently being handled by another attorney or agent. The first page has signature lines for the Assignee (if any), and up to two inventors. The second page is required if there are more than two inventors. Please consider whether you should ask the Firm to complete a conflict of interest evaluation on the application before this form is signed.

Assignments by the Inventor(s)
A written assignment from each inventor must be recorded at the U.S. Patent and Trademark Office if the employer wants to own the patent application, generally speaking. Simple signature is sufficient to be recorded, but witnesses or notarization may be advisable under certain circumstances. Therefore, three versions of the form appear below. For each version, the supplemental page is required if there is more than one inventor. If an inventor is deceased, incapacitated, or refuses to sign, then a Petition to the Commissioner may be drafted based on the circumstances of the case.

Assignment
If the assignee of a U.S. application or registration has an address
outside the U.S., then a Power of Attorney and Designation of Domestic Representative form is required for each application/registration in the Assignment, generally speaking. This Assignment form is for recordal and does not contain a number of common provisions (such as a warranty of title clause, an integration clause, and a notarization clause) which may be advisable depending on the circumstances. The U.S. Patent and Trademark Office will record an Assignment that is signed only by the Assignor, and not by the Assignee. An effective date may be added to the first sentence of this form, if appropriate (for example, "This Assignment is made effective January 1, 2001, between ....").
